Building a remote patient monitoring program with Apple and Jamf

Bridging the gap between hospital and home is key to patient success, and a necessary way to transform patient care. Patients leave an environment that is well controlled and monitored, to their home, at times with only a binder and pen to record their symptoms and outcomes. And leaving the hospital is scary, for patients, caregivers, and parents, too. So what kinds of ways can we address this problem?

The Locus platform is designed to close the gap in how patients transition to home after a hospitalization. A simple iPad based solution that would support a patient in their home environment and extend care beyond the hospital walls.
