Bridging the gap between hospital and home is key to patient success, and a necessary way to transform patient care. Patients leave an environment that is well controlled and monitored, to their home, at times with only a binder and pen to record their symptoms and outcomes. And leaving the hospital is scary, for patients, caregivers, and parents, too. So what kinds of ways can we address this problem?
